Spiritual Direction Completion Form

To ensure that the Presbytery of San Francisco assist with the cost of Spiritual Direction sessions, we ask that both the spiritual director and the spiritual directee complete the form after each meeting. Once the Presbytery receives both forms, we will process the payment to the Spiritual Director. Thank you!

If multiple sessions are being submitted at once, what is the total amount paid? The spiritual directee should pay for half of the session and the Presbytery will pay the other half of the session directly to the spiritual director.
